The following information and documents are required for school enrollment:

1) Proof of Residency

Parent or legal guardian of a new student must present one of the following verifications as proof of residence:

___ Escrow papers.
___ Lease Agreement/Rental Contract
___ San Diego Gas & Electric Bill
___ Pacific Bell Bill
___ Water Bill
___ Cox Cable Bill
___ Dept. Social Services Notice of Action

Falsification of any information or documents, either written or verbal, relative to this
Verification procedure will result in revocation of enrollment of the student.

2) Immunizations

Complete immunization records including, month, year and doctor/clinic stamp, of all
inoculations:

STUDENTS CANNOT BE ENROLLED IN SCHOOL UNTIL PROOF OF ALL
REQUIRED IMMUNIZATIONS IS PROVIDED.

3) Dental/Physical Examine

Each incoming Kindergartner will need to have a complete physical exam as well as
dental exam. Forms can be picked up in the Nurse’s Office.

4) Birth date Verification

A certified copy of the child’s birth date is required to enter Kindergarten, First Grade
and all Initial Enrollees

Padre/tutor de un nuevo estudiante debe presentar uno de los siguientes comprobantes de domicilio:

___ Título de propiedad
___ Contrato de renta
___ Una factura de la luz, agua, teléfono o cable
___ Una notificación del departamento de servicios sociales

Falsificación de información de uno de estos documentos, por escrito o verbal,
resultará en revocar la matriculación del estudiante.

2) Vacunas

El record de vacunas completamente lleno y que incluye fecha, firma del doctor o
sello de la clínica de todas las vacunas recibidas.

¡TODO ESTUDIANTE QUE NO TENGA TODA LAS VACUNAS NECESARIOS,
NO PODRA CONTINUAR CON LA MATRICULACÍON!

3) Examen físico y examen dental

Cada niño de Kinder debe de tener un examen físico y tambén un examen dental.
Por favor de recoger las formas en la Oficina de la Enfermera.

4) Verificación de fecha de nacimiento

Los niños de kinder y primer grado necesitan presentar su acta de nacimiento original.

School Site Council (SSC)

The SSC is an elected decision-making group comprised of parents, community members, site administrators, teachers and other staff. At the secondary level, students are included. The SSC has an ongoing responsibility to develop, implement, monitor and evaluate the site plan. The SSC also oversees all the categorical funds such as Title I, Second Language and GATE. All schools are required to have an SSC. Contact your site administrator or SSC chair if you are interested in joining.

Site Governance Team (SGT)

Each school has a SGT to act as its advisory body on issues other than major categorical programs. Contact your school administrator if you are interested in being part of your school's SGT.

English Learner Advisory Committee (ELAC)

Schools which have twenty-one or more English as a Second Language (ESL) students also have an English Learner Advisory Committee. It serves as the advisory body to the school on four key issues:

• The school plan for English learners
• Needs assessment
• Administration of the language census
• Efforts to make parents aware of the importance of attendance
